The Shri Vishnu Sahasranama Stotram is the hymn of the thousand names of Lord Vishnu. It comes in the Anushasana Parva of the Mahabharata as a dialogue between Bhishma and Yudhishthira: after the great war, Yudhishthira asks Bhishma, who lies on a bed of arrows, which god is the greatest and by the japa of what a being is freed from the bonds of worldly life, and Bhishma answers with the thousand names of Vishnu. The Mahabharata is traditionally attributed to the sage Vedavyasa, and one shloka of the hymn (141) itself says that it was sung by Vyasa.
It is among the most widely read of all stotras: in daily worship at home, on Thursdays and Ekadashi, in group recitations at temples, and in Jyotish as a traditional remedy for Jupiter and Mercury. On this page the full text is taken letter for letter from the Gita Press, Gorakhpur booklet Shri Vishnu Sahasranama Stotram and checked against the Gita Press Sahasranama Stotra Sangrah. With it come the meaning of every shloka of the dialogue and the phalashruti, how the names are read with the meaning of some well-known names, how to read the hymn, and a free PDF after sign-in.

Salutation to Vishnu, the Prabhavishnu (all-powerful, lord over all), by the mere remembrance of whom a person is freed from the bondage of birth and samsara (the round of births and deaths).

Salutation to Vishnu the all-powerful (Prabhavishnu), the first of all beings and their origin, the upholder of the earth (Bhubhrit), whose form is the many forms of the world.

Vaishampayana (Vyasa's disciple, who is telling the Mahabharata to King Janamejaya) said: having heard all the dharmas in full, and every teaching that purifies, Yudhishthira spoke again to Shantanava (Bhishma, son of Shantanu).

By praising whom, and by worshipping whom, do people gain what is good?

भीष्म ने कहा: जो मनुष्य सदा तत्पर रहकर जगत् के प्रभु, देवों के देव, अनन्त (जिनका अन्त नहीं) पुरुषोत्तम (परम पुरुष) की सहस्र नामों से स्तुति करता है (यह वाक्य श्लोक 6 में पूरा होता है),Bhishma said: the person who, ever intent, praises with the thousand names the lord of the world, the god of gods, the endless (Ananta) Purushottama (the supreme Person) (the sentence ends in shloka 6),

and who daily praises Vishnu, who has neither beginning nor end, the great lord of all the worlds and their overseer (Lokadhyaksha), goes beyond all sorrow.

Shloka 8
In my view this is the greatest of all dharmas: that a person should always worship Pundarikaksha (the lotus-eyed Vishnu) with devotion, through hymns of praise.

Where it comes from
The Vishnu Sahasranama is in the Anushasana Parva, the thirteenth book of the Mahabharata. After the war at Kurukshetra, Bhishma lies on a bed of arrows awaiting the sun's northward course (uttarayana), and Yudhishthira has already heard from him the teachings on the duties of kings, on conduct in distress, on liberation and on giving. Then he asks the questions with which this hymn begins (shlokas 2 and 3). The whole story is being told by Vaishampayana, so the first shloka opens with 'Vaishampayana said'.
The colophon at the end of the booklet gives its place: in the Mahabharata, Vyasa's collection of a hundred thousand verses, in the Anushasana Parva, in the dialogue of Bhishma and Yudhishthira, the divine thousand-name hymn of Shri Vishnu. The colophon is not part of the hymn, so it is not set among the shlokas.
The text here follows the small Gita Press booklet: two opening shlokas of salutation, 13 shlokas of dialogue, 107 shlokas of names (14 to 120), one short line closing the names, and 22 shlokas of phalashruti (121 to 142). Many editions add a viniyoga, nyasa and dhyana verses (such as 'Shantakaram bhujagashayanam') before the names, and a few more shlokas at the end; this booklet does not print them, so they are not here. Those whose tradition reads them can add them before the names.
How the names are read
The thousand names are in shlokas 14 to 120, 107 shlokas in all. They begin with 'Om vishvam vishnur vashatkaro' and end with 'sarvapraharanayudhah'; the short line after them, 'sarvapraharanayudha om nama iti', repeats the last name as an address with a salutation and closes the list. In the stotra the names stand in the nominative case, joined by sandhi in the rhythm of the anushtup metre, so they are read straight through like any shloka, not one name at a time. These shlokas carry no separate meaning; the meaning of some well-known names is given below.
How the 107 shlokas yield exactly a thousand names is settled by the commentarial tradition, since some names occur more than once (Vishnu, Achyuta, Padmanabha, Keshava, Vasudeva) and some long compounds count as one name. The classic commentary is the Vishnu Sahasranama Bhashya traditionally attributed to Adi Shankaracharya, which divides the whole text into exactly a thousand names and explains a repeated name in a different sense at each place. A namavali is built on that division: each name is said on its own in the dative case with 'Om' and 'namah', as in 'Om vishnave namah' and 'Om vashatkaraya namah', and in archana a flower, tulsi leaf or grain of rice is offered with each name. This page gives the stotra only, not a namavali.
Some well-known names and their plain sense: Vishvam (shloka 14, the first name): the one who is the whole universe. Vishnu (14): the one who pervades all. Vashatkara (14): vashat is the call uttered as an offering is poured into the sacred fire; the name shows him as the sacrifice itself and its lord. Bhutabhavyabhavatprabhu (14): lord of what was, what will be and what is. Keshava (16): the one with beautiful hair; tradition also reads it as the slayer of the demon Keshi. Purushottama (16): the highest of all beings, the supreme Person.
Padmanabha (19): from whose navel rose the lotus on which Brahma sits. Achyuta (24): the one who never falls from his own nature. Janardana (27): the one to whom people (jana) pray for their good; ardana also means to afflict, so he is also the one who punishes the wicked. Govinda (33): the keeper of cows; go also means the earth and speech, and commentators read the name in those senses too. Narayana (39): the one whose dwelling (ayana) is the waters, or all beings (nara). Vasudeva (49): the son of Vasudeva, and the god who dwells in all and in whom all dwell.
How to read it
Bathe, put on clean clothes, sit on a mat facing east or north, and light a lamp before an image of Vishnu or Krishna or a shaligram; an offering of tulsi leaves is held dear in Vishnu worship. Read the two opening shlokas and the dialogue (1 to 13) first, then the names (14 to 120), and the phalashruti (121 to 142) last. It is thought best to read the whole text at one sitting, without getting up or talking in between.
If Sanskrit is unfamiliar, read slowly and clearly. In the name shlokas the words are joined by sandhi (as 'vishnur vashatkaro' joins Vishnu and Vashatkara); read them as printed and do not stop halfway through a word. Reading for a few days alongside someone who knows the text, or with a careful recording, settles the pronunciation quickly.
The phalashruti (shloka 122) speaks of both the one who hears and the one who recites, so those who cannot read it themselves may listen with faith. In temples and homes many people also read it together as a group.
Meaning and fruit
Yudhishthira's questions (shlokas 2 and 3) ask one thing from several sides: who is the one god, what is the highest refuge, by praising and worshipping whom good comes, which dharma is the greatest, and by the japa of what a being is freed. Bhishma's answer (shlokas 4 to 13) is that praising and worshipping with devotion Vishnu, who has no beginning or end, is the greatest dharma (shloka 8), and he then recites the thousand names of that Vishnu.
The phalashruti (shlokas 121 to 133) says that no ill comes here or hereafter to one who hears or reads it daily; that each person gains dharma, wealth, their wishes or children according to what they seek; that the reader gains fame, lasting prosperity, health and radiance, and is freed from illness, bonds, fear and trouble; and that they cross over hardships and reach the eternal Brahman. These are the words of the phalashruti, and tradition links them to reading done with faith and discipline.
The last shlokas (134 to 140) go beyond the fruits to the heart of the hymn: the sky, the earth and the ocean, gods and asuras, the senses, mind and intellect, the Vedas and the shastras all come from Vasudeva and are held up by his power; the one Vishnu pervades the three worlds as the many beings. So the hymn is read not only for its fruits but also as a practice of seeing Vishnu in all things.

In Jyotish the Vishnu Sahasranama is tied to both Mercury and Jupiter. The Brihat Parashara Hora Shastra names Vishnu as the deity of Mercury, and its chapters on antardasha results give the recitation of the Vishnu Sahasranama to calm the adverse results of Mercury's antardasha in several mahadashas, Mercury's own among them; Thursday is the day of Vishnu worship, and reading it for Jupiter is common practice too. It is given as a traditional remedy when Mercury is debilitated (in Pisces), combust or afflicted by malefics, when Jupiter is debilitated (in Capricorn) or joined with Rahu, and during the mahadasha or antardasha of Mercury or Jupiter, especially when studies, decisions, speech, religious life or peace of mind meet obstacles. Where Mercury and Jupiter sit in your chart and which dasha is running can be seen by making your kundli on this site.
