The Shiv Chalisa is a hymn to Lord Shiva in Awadhi-coloured Hindi: one opening doha, forty-one chaupais and two closing dohas. Its author is Ayodhyadas, who names himself in both the opening and the closing dohas. The verses first describe Shiva's form, then recall his deeds: the slaying of Taraka, Jalandhara and Tripura, the drinking of the poison at the churning of the ocean, and Rama's worship with a thousand lotuses. They end with the devotee's plea and the fruits of recitation.
It is read in homes on Mondays, on Pradosh, on Shivratri and through the month of Shravan, and people take it up as a daily rule in times of trouble, illness, debt or the wish for a child. The text here is transcribed letter by letter from a printed Haridwar edition. Under every doha and chaupai is a plain meaning, and after sign-in the whole text can be downloaded as a free PDF.

In the month of Margashirsha, on the sixth day, in the hemant season of samvat sixty-four, this Chalisa in praise of Shiva was completed, and it brought blessing.
How to read it
Bathe in the morning, put on clean clothes and sit before a Shivalinga or a picture of Shiva and Parvati. Offer water, bel leaves, incense and a lamp. The Chalisa itself says: offer incense, lamp and food, and recite before Shankara.
Begin with the opening doha to Ganesha, then the forty-one chaupais, and close with the two final dohas. It is customary to follow the reading with the japa of Om Namah Shivaya and the Shiva aarti.
Faith and attention matter more than the count. Those who take a vow read it once, three, five or eleven times; in trouble the custom is eleven readings on a Monday, and a daily reading through Shravan.
Meaning and fruit
In its closing verses the author himself lists the fruits of this Chalisa: the one who reads it with a devoted mind has Shambhu's help, the debtor is freed, the childless find a child, bodily affliction leaves, and the sins of many births are cut away.
The middle verses tell the stories of Taraka, Jalandhara, Tripura, the poison of the churning of the ocean and Rama's worship with a thousand lotuses. They remind the reader that Shiva comes when called in distress and that he tests his devotee before granting the boon. That is why this Chalisa is read above all in fear, illness and trouble from enemies.
Beyond faith there is a plain benefit too: a few minutes each day with the mind held on one text steadies it, and the day begins from a settled place.
